You are registering for certification as a Certified Personal Medicine Coach. By clicking “I have read and agree to the terms and conditions of this page as follows” you are agreeing to the following terms:

1. You confirm you have access to reliable internet and a webcam
   
   

2. You confirm you are able to attend all of the scheduled Live Coaching Calls/Sessions
   
   
3. You understand you must meet all requirements set forth in the course syllabus to be eligible for Certification.

    

4. Once you have become a Certified Personal Medicine Coach, Pat Deegan, PhD. & Associates, LLC (“PDA”) will grant you a limited license to copy, distribute and use materials developed and provided by PDA, including copyrighted worksheets, Manuals, Guides, Personal Medicine Cards and/or videos (“PDA Materials”), in accordance with PDA policies, in your work with individuals you are personally coaching and supporting. This license will end if you do not maintain your certification.
   
   

5. Except as provided above, you may not copy, duplicate, post to social media/internet/websites, or otherwise distribute any PDA Materials, and you may not use the PDA Materials for any other purpose, without PDA’s permission. PDA Materials will remain the intellectual property of PDA.
   
   

6. When you perform coaching as a Certified Personal Medicine Coach or use PDA techniques or methods in any other way, you will not be acting as an employee, contractor, partner or agent of PDA, and PDA will have no liability to you, the individuals you support or train, or any other party for your actions or inactions in connection with such coaching, training or use.
